For 10th Class:

1.             Age proof
 
2.             School leaving certificate
 /T.C/ Date of Birth certificate
3.             Last year passed or failed certificate ( any one of them)
4.             passport size photo

For 12th Class:

1.            Photo copy of 10th class mark sheet
2.            Address proof  ( Addhar card /Voter card/Rashan card/Rent agreement)
3.            Two passport size photo
